Key Responsibilities:

Deliver high-quality in-home skilled nursing care and interventions Complete thorough patient assessments and accurate, timely OASIS documentation Collaborate effectively with physicians and interdisciplinary care teams to design and deliver individualized care plans Provide wound care, manage medications, and oversee chronic disease monitoring and patient education Report patient condition changes and care updates promptly to the clinical team

Qualifications:

Active RN license At least one year of recent home health RN experience Proficient in OASIS documentation (required) Experience with HomeCare HomeBase (strongly preferred) Reliable transportation and valid driver's license Comfortable managing a consistent daily visit schedule
