Requirements:

2 years of experience as a RN in Home Health setting; Valid RN license; Be willing to obtain Georgia licensure. HomeCare HomeBase EMR highly preferred

Responsibilities:

Provide skilled nursing care to home health patients in their residences. Develop and implement patient care plans in collaboration with healthcare team members.
