Job Description

Position requires an active nurse license in the State of Iowa. Offering this opportunity to an RN or an experienced LPN. Monday- Friday 8am-4pm or 8am-5pm •you decide ( Hours may vary based on facility need) Active participate with the on-call rotation (Friday-Sunday) - incentive based on-call . On-call rotation includes every 5th weekend, does not include every other holiday. The MDS/Care Plan Coordinator is responsible for completing MDS documents and preparing resident plans of care timely and efficiency. The MDS/Care Plan Coordinator is responsible for conducting quality audits and reporting results to the DON. This position includes the tasks and duties of the center's skin nurse which includes weekly skin assessments . Excellent Communication Skills required. Experience Based Pay. On average the MDS Coordinator would complete a total of 5 MDS a week (includes a variety of Quarterly, Annual, Significant Changes and Admission) Summary As an MDS Coordinator, you will be instrumental in developing and managing comprehensive care plans for residents, ensuring adherence to regulatory standards. Reporting to the Director of Nursing, you will utilize your core nursing skills, including patient assessment and care, alongside premium skills in EHR systems. Your expertise in long-term care and critical care will enhance the quality of services provided.
