This position requires the willingness to travel for in person assessments.

Pay structure:

$75/hr for assessments, $20/hr for drive time, and $0.58/mile for mileage Homecare Hub of Iowa d/b/a Home Halo of Iowa is a non-medical home care agency. Our clients will receive a RN assessment visit at admission and a re-assessment visit on an ongoing basis. The RN will travel to client homes on an as needed, monthly, or quarterly basis to assess the current non-medical care needs of the client and to prepare a care plan to be implemented by the non-medical care team.

Responsibilities:

• New and existing client visits including assessment, planning, and evaluation of patient's condition
• Collaborate with interdisciplinary healthcare team to ensure quality client care
• Maintain accurate and up-to-date client records

Experience:

• Familiarity with HIPAA regulations and patient privacy practices
• Experience using electronic medical record systems
• Previous experience in home care or community health is a plus

Qualifications:

• Valid Registered Nurse license in the state of Iowa
• Excellent communication and interpersonal skills
