A Hospice Registered Nurse provides care to patients who are terminally ill or at the end of their lives. Provides care needed to keep patients comfortable, and provides clinical and emotional support for family caregivers. Works in hospice facilities and private homes.
Join our compassionate team as an LPN/RN Case Manager! You'll be the heart of hospice care, coordinating care for patients and families during their most important time. This position is part-time weekends, particularly Sunday. Open for PRN throughout the week as well.
What You'll Do:
Manage patients, ensuring they receive appropriate care. Complete assessments, create care plans, and document care. Collaborate with doctors, social workers, and other team members. Provide support and education to families. Enjoy flexible scheduling and work-life balance (avoid burnout!).
We're Looking For:
Idaho RN/LPN License. 2+ years of experience (pref. hospice). Strong communication and teamwork skills. Valid driver's license & reliable transportation. Certifications, Licenses and Registrations Registered Nurse licensed in the state of employment. Minimum of one (1) year nursing experience, preferably in a Home Health/Hospice setting within the past five (5) years is preferred. Certification in Hospice and Palliative Nursing a plus but not required. Current CPR certification. Must have a valid driver's license and auto liability insurance. Other Skills and Abilities Understanding and acceptance of hospice philosophy. Strong clinical, organizational and interpersonal skills. Strong written and oral communication skills.
