Registered Nurse (RN) Case Manager

This position pays up to $46 per hour DOE Join our compassionate team as a RN Case Manager! You'll be the heart of hospice care, coordinating care for patients and families during their most important time. What You'll Do:

Manage 10-14 patients, ensuring they receive appropriate care. Complete assessments, create care plans, and document care. Collaborate with doctors, social workers, and other team members. Provide support and education to families. Enjoy flexible scheduling and work-life balance (avoid burnout!).

We're Looking For:

Idaho RN License. 2+ years of experience (pref. hospice). Strong communication and teamwork skills. Valid driver's license & reliable transportation.
