Job Description

Nursing degree, active Nurse Practitioner (NP) license and minimum 1+ years Home Health Nurse Practitioner (NP) experience required. Applicants who do not meet these qualifications will not be considered. Experienced nurse practitioners with a passion for delivering patient-centered care are needed for per diem home health risk assessments throughout Shoshone County, ID. In this role, you will visit Medicare Advantage plan members in their homes, providing important annual health assessments and post-discharge visits. Each visit takes approximately 45 minutes, and you may be scheduled in locations such as Kellogg, Pinehurst, or Osburn.
Schedule:
Flexible shifts available Monday through Sunday Set your own schedule to accommodate personal and professional commitments Maintain a minimum weekly or bi-weekly hourly commitment with autonomy
Key Responsibilities:
Complete comprehensive annual risk assessments for Medicare Advantage plan participants at their residence Thoroughly review prior and current health histories, existing medical conditions, and medication usage Offer education regarding health conditions and medication management to patients Identify gaps in care, making recommendations to address these issues No diagnosis, treatment planning, or medication prescribing necessary
Qualifications:
Active Nurse Practitioner license in Idaho required Board certification through AANP or ANCC Current CPR certification (BLS) MSN degree from an accredited nursing program At least 6 months of recent Nurse Practitioner experience preferred Per evaluation, this per diem opportunity offers competitive compensation between $80 and $90, along with a mileage stipend and compensation for administrative time.
