Oak Street Health is seeking a Registered Nurse to deliver compassionate, team-based primary care to adults on Medicare in underserved communities. In this role, you'll provide assessments, triage, chronic disease management, vaccinations, and patient education while collaborating closely with physicians, social workers, and care managers.
