Job Description

Oak Street Health is seeking a Registered Nurse to deliver compassionate, team-based primary care to adults on Medicare in underserved communities. In this role, you'll provide assessments, triage, chronic disease management, vaccinations, and patient education while collaborating closely with physicians, social workers, and care managers. You'll use evidence-based protocols and an EHR to coordinate care, support population health initiatives, and improve outcomes. Responsibilities Provide comprehensive primary and preventive nursing care to adult Medicare patients in a clinic setting Conduct patient assessments, obtain histories, and monitor chronic conditions Administer medications, treatments, and vaccines following clinical protocols Collaborate with physicians, care teams, and community partners to coordinate care plans Educate patients and families on disease management, medications, and healthy lifestyles Document all care accurately in the electronic health record (EHR) Participate in quality improvement and population health initiatives Support care transitions, follow-up outreach, and care coordination for high-risk patients Required Skills Patient assessment Chronic disease management Medication administration Care coordination Electronic Health Records (EHR) Triage and clinical decision-making Vaccination and injections Patient and family education Infection control practices Vital signs monitoring and documentation
