Role Overview:
Perform comprehensive, in-home health risk assessments, each averaging 45 minutes Assess members in varied locations within St. Clair Countyincluding Belleville, OFallon, and East St. Louisbased on shift assignments Review medical histories, current health conditions, and medication usage Provide education on conditions and medications to patients and families Spot potential gaps in care and recommend actionable steps for improvement No requirement to create treatment plans or prescribe medications
Required Qualifications:
Active Nurse Practitioner license in Illinois Board Certification (AANP or ANCC) MSN degree from an accredited nursing program Current CPR certification (BLS) At least six months of NP experience preferred This opportunity offers compensation of $80-$90 per evaluation, with additional mileage stipend and payment for administrative time.
