Key Responsibilities:
Perform detailed annual, post-discharge, or periodic in-home health risk assessments (averaging 45 minutes per visit) Review and document medical histories, current conditions, and medication usage Deliver patient education on medical diagnoses and medications, ensuring understanding and compliance Identify care gaps and provide recommendations for follow-up (no treatment plan or prescriptions required) Support member health by offering insight and community resources when necessary
Desired Qualifications:
Unrestricted Nurse Practitioner (NP) license in Illinois Master of Science in Nursing (MSN) from an accredited institution Board Certification from AANP or ANCC Active Basic Life Support (BLS/CPR) certification At least 6 months of recent NP experience preferred Strong communication, assessment, and documentation skills Those selected will receive $80-$90 per completed evaluation, plus mileage reimbursement and compensation for administrative time.
