only - applicants must be willing to commit to a Full-Time schedule of 40 hours per week. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation Administer medications and injections accurately, including IV infusions and medication management for acute pain, hospice, or palliative care patients. Monitor patients' conditions closely through telemetry and other advanced equipment, responding promptly to changes in vital signs or clinical status. Provide specialized care in areas such as critical care, trauma medicine, neuro ICU, or long-term care settings, including memory care and behavioral health support. Assist with diagnostic procedures like catheterizations, suturing, laboratory specimen collection and processing, and imaging procedures such as sonography or CT scans. Collaborate with interdisciplinary teams to develop discharge plans, coordinate case management efforts, and ensure compliance with HIPAA regulations and NCQA standards. Maintain accurate medical documentation of patient interactions, treatments administered, and clinical observations to ensure seamless continuity of care. Qualifications Valid RN license with current registration; Proven experience working in hospital medicine, SNF & ALF settings Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and aseptic techniques required for safe residentcare. Experience working with senior patient populations. Critical thinking skills with the ability to perform triage assessments and respond effectively to urgent situations in fast-paced environments such as urgent care or hospital settings. Excellent communication skills coupled with a compassionate approach to patient-centered care; ability to work independently during night shifts while maintaining team collaboration.
