Job Description

Take on a rewarding opportunity making an impact in patient health by performing home health risk assessments for Medicare Advantage plan members in Stephenson County. This per diem position as a Nurse Practitioner allows you to support community members right where they live, with a flexible schedule and the ability to create your own shifts.
Schedule:
Flexible shifts available between Monday and Sunday Set your own schedule Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
Conduct comprehensive home health risk assessments, generally lasting 45 minutes, for members residing in areas including Freeport, Lena, and Orangeville Review and evaluate prior and current medical history, conditions, and medication usage Educate patients about their health conditions and medications to empower better self-management Identify care gaps and suggest recommendations for closing those gaps Focus exclusively on assessments; no treatment planning or medication prescribing involved
Qualifications:
Active Nurse Practitioner license in Illinois Graduate of an accredited MSN program Current Board Certification (AANP or ANCC) CPR (BLS) certification Minimum of 6 months NP experience preferred In this role, compensation is provided at $80-$90 per evaluation, supplemented by a mileage stipend and recognition for administrative time.
