Long-Term Care / Skilled Nursing Facility Registered Nurse Shift - Days (7:00A - 7:30P) Job Summary Join our dedicated healthcare team as a Long-Term Care / Skilled Nursing Facility Registered Nurse, where your expertise will directly impact the quality of life for residents in a compassionate, dynamic environment. This position offers an exciting opportunity to utilize your diverse clinical skills in a rewarding long-term care setting. Duties Deliver direct patient care by assessing residents' health status, administering medications, and performing routine physical examinations with precision and compassion Develop and implement individualized care plans in collaboration with interdisciplinary teams, ensuring adherence to best practices and regulatory standards. Monitor vital signs, manage acute pain, and respond swiftly to emergent situations using critical care techniques including airway management and ventilator support Conduct comprehensive patient assessments utilizing advanced EMR/EHR systems to document medical records accurately and efficiently Perform specialized procedures while maintaining aseptic technique Coordinate discharge planning, case management activities, and family education to support ongoing resident well-being and transition of care Requirements Valid RN license in the state of IL. Current BLS certification At least 2 years of RN experience At least 1 year of
LTC / SNF
