PRN as neededSalary Range:
$36.00- $40.
Your responsibilities include:
Delivering professional nursing care across the full nursing process within your scope of practice Conducting thorough patient and family assessments and contributing to individualized care plans Completing all visit types as needed to meet patient and family needs Administering medications and treatments per physician orders Identifying nursing diagnoses from health assessment data and responding accordingly Recording accurate, timely clinical notes including pain and symptom management outcomes Communicating changes in patient condition to physicians and obtaining orders as required Coordinating with community health partners to support seamless care transitions Ensuring equipment and necessary services are arranged and available for patients Maintaining and growing your clinical skills through continuing education and in-service participation Benefits We Offer Competitive pay Company provided equipment Paid Time Off Why Choose Transitions Hospice? What You Bring Active RN license in the state of employment Graduate of an accredited school of Nursing. Clinical experience in hospice, home health, or a related setting preferred Flexibility and confidence working across varied patient assignments Strong assessment skills and sound clinical judgment under pressure Excellent documentation habits and communication skills Valid driver's license and willingness to travel locallyBenefits
