Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage members Review patients prior and current medical history, conditions, and medication usage Educate members on their health conditions and proper medication management Identify gaps in care and recommend solutions to promote optimal health outcomes No direct treatment planning or medication prescribing required

Qualifications:
