Job Description

Take on a rewarding per diem Nurse Practitioner role specializing in home health risk assessments within St. Clair County, Illinois. This position centers on providing vital Medicare Advantage plan member evaluations in the community, including areas such as Belleville, OFallon, and East St. Louis. Tailor your shifts to your availability with a fully flexible schedule, allowing you to create your own hours and manage a minimum weekly or bi-weekly hourly commitment.
Schedule:
Flexible shifts available Monday through Sunday Customize your own work schedule Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
Conduct annual home health risk assessments for Medicare Advantage members in their homes Review and evaluate prior and current medical history, existing conditions, and medication usage Provide education to patients regarding health conditions and proper medication management Identify care gaps and make recommendations for further medical follow-up or services Focus strictly on assessment: no requirement to prescribe medications or initiate treatment plans
Qualifications:
Active Nurse Practitioner license in Illinois Current CPR certification (BLS) Board Certification (AANP or ANCC) Minimum of six months recent Nurse Practitioner experience preferred MSN degree from an accredited nursing program This per diem position provides a competitive earning opportunity, with compensation of $80-$90 per evaluation, plus mileage stipend and administrative time.
