MDS Coordinator Job Summary:
Northview Health and Living is seeking a dedicated and compassionate MDS Coordinator to join our team in Anderson, IN. As an MDS Coordinator, you will play a crucial role in ensuring the accuracy and timeliness of Minimum Data Set (MDS) assessments, care plans, and quality measures for our residents. Coordinate and conduct MDS assessments, care plans, and quality measures for residents in our skilled nursing facility Ensure timely and accurate completion of MDS assessments and care plans, meeting established deadlines and quality standards Collaborate with interdisciplinary teams, including physicians, nurses, and therapists, to develop and implement individualized care plans Maintain accurate and detailed records of resident assessments, care plans, and quality measures Analyze data and identify trends to improve care processes and outcomes Provide education and training to staff on MDS and care planning processes Participate in quality improvement initiatives and quality management activities Ensure compliance with federal, state, and accrediting agency regulations and standardsRequirements:
Minimum Requirements:
Minimum 2 yrs of experience. Must be an LPN Strong organizational and communication skills Ability to work effectively in a fast-paced environment Strong attention to detail and accuracy Ability to work collaboratively with interdisciplinary teamsAbout Northview Health and Living:
