Clinical Nurse - Direct Primary Care Carpenter Family Care | Batesville, Indiana Carpenter Family Care is a small, patient-centered direct primary care practice looking for a dependable, compassionate, and clinically strong RN or LPN to join our team. We are a science- and evidence-based practice with a strong emphasis on prevention, lifestyle medicine, and personalized primary care. We believe nutrition, physical activity, sleep, stress management, and other lifestyle factors play an important role in health and disease prevention. We provide evidence-based recommendations for preventive care. This is an excellent opportunity for a nurse who enjoys developing meaningful relationships with patients, working closely with a physician, and having a varied role that combines clinical care, patient communication, triage, care coordination, and practice support. Clinical Responsibilities Room and check in patients Obtain and document vital signs Obtain focused histories and medication reconciliation Perform phlebotomy and specimen collection Perform EKGs and point-of-care testing Administer medications within scope of practice Assist with procedures and examinations Prepare clinical rooms and maintain medical supplies Provide patient education and follow-up Assist with chronic disease management and preventive care Follow established clinical protocols and standing orders Communicate clinical findings and concerns to the physician Patient Triage & Communication A key component of this position will be serving as an initial clinical point of contact for patients by phone and electronic messaging.

Responsibilities include:

Answer patient calls and portal messages Obtain appropriate history and clinical information regarding patient concerns Perform protocol-based nursing assessment and triage Determine the appropriate level and urgency of care within scope of practice and established protocols Assist with same-day and follow-up patient needs Communicate clearly and compassionately with patients Help manage patient communication when the physician is out of the office Care Coordination Follow up on laboratory and diagnostic testing as directed Communicate appropriate results and physician recommendations to patients Assist with referrals and specialist coordination Obtain outside medical records Communicate with pharmacies, specialists, hospitals, laboratories, and imaging centers Assist with post-hospital and post-ER follow-up Assist with medication refills according to established protocols Preventive & Lifestyle Medicine Support the practice's emphasis on prevention and healthy lifestyle Reinforce physician recommendations regarding nutrition, physical activity, sleep, stress management, and other healthy lifestyle behaviors Administrative & Practice Responsibilities Process prior authorizations Assist with referrals and insurance-related tasks Complete forms and clinical paperwork Maintain accurate and timely documentation in the electronic medical record Assist with scheduling and patient follow-up Maintain a clean, organized, and welcoming clinical environment Monitor and restock medical supplies Assist with other day-to-day needs of a small medical practice Work collaboratively with the physician and other members of the team Qualifications Current Indiana RN or LPN license Previous outpatient, primary care, urgent care, or family medicine experience preferred Strong clinical assessment and communication skills Excellent phlebotomy skills Comfortable performing patient triage by phone and electronic messaging Able to recognize clinical situations requiring physician involvement or urgent evaluation Able to work independently while recognizing when to seek assistance Excellent organization and attention to detail Dependable and responsible Strong interpersonal and communication skills Comfortable working in a small, collaborative practice Experience with an electronic medical record preferred Interest in preventive and lifestyle medicine Basic knowledge of healthy lifestyle practices, including nutrition, physical activity, sleep, and stress management Commitment to science- and evidence-based healthcare Comfortable discussing evidence-based preventive care and vaccination recommendations with patients What Makes Our Practice Different At Carpenter Family Care, we take the time to get to know our patients and focus on the whole person rather than simply treating individual symptoms.
