In this role, you will conduct comprehensive risk assessments for Medicare Advantage members in diverse areas across Parke County, including annual evaluations and post-discharge visits. Each home assessment averages 45 minutes and focuses on complete, patient-centered reviews, allowing you to make a lasting impact while maintaining a flexible work-life balance.

Key Responsibilities:

Perform thorough in-home risk assessments for Medicare Advantage members Review past and present medical history, assess medical conditions, and evaluate medication regimens Educate patients on health conditions and medication management Identify potential care gaps and recommend actionable solutions No development of treatment plans or medication prescribing required

Qualifications:

Active Nurse Practitioner license in Indiana Current BLS (CPR) certification Board Certification through AANP or ANCC Minimum of 6 months NP experience preferred Masters degree in Nursing (MSN) from an accredited program This position offers a competitive compensation structure, with a per-evaluation payment ranging from $80 to $90, along with a mileage stipend and payment for administrative time.
