Job Description

Deliver meaningful care in the community by conducting home health risk assessments as a Nurse Practitioner on a per diem basis. This dynamic opportunity allows you to work with Medicare Advantage plan members throughout Clay County, visiting individuals in their homes in locations such as Brazil, Clay City, and Knightsville.
Schedule:
Flexible shifts available Monday through Sunday Design your own schedule to suit your needs Minimum weekly or bi-weekly hour commitment
Key Responsibilities:
Complete comprehensive annual home health risk assessments for plan members Review prior and current medical histories, conditions, and medication usage Educate patients and families about medical conditions and medications Identify care gaps and recommend solutions, ensuring members receive optimal care No requirement for treatment plan development or prescribing medications
Qualifications:
Active Nurse Practitioner license (IN) Board Certification via AANP or ANCC Current CPR (BLS) certification MSN from an accredited nursing program At least 6 months of recent NP experience preferred This role offers a competitive compensation structure of $80-$90 per assessment, along with a mileage stipend and administrative time paid.
