Responsibilities:

Complete annual and post-discharge home health risk assessments for Medicare Advantage members Review current and prior medical history, conditions, and medication usage Deliver patient education on diagnoses and medication regimens Identify care gaps and offer recommendations to help address them Documentation and assessment onlyno requirement for prescribing or developing treatment plans

Qualifications:

Active Nurse Practitioner license in Indiana MSN from an accredited nursing program Board certification through AANP or ANCC Current BLS/CPR certification At least 6 months of nurse practitioner experience preferred Compensation for this role ranges from $80-$90 per evaluation and includes a mileage stipend, as well as compensation for eligible administrative time.
