Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members Review and evaluate medical history, chronic conditions, and current medications Deliver education and guidance regarding health conditions and prescription usage Detect gaps in care and offer recommendations for follow-up No treatment plan development or medication prescribing involved

Qualifications:

Active Nurse Practitioner (NP) license in Indiana Board Certification through AANP or ANCC Current CPR certification (BLS) MSN from an accredited nursing school At least 6 months of recent NP experience is preferred Nurse Practitioners in this role receive competitive pay between $80 and $90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
