Job Description

Seeking a dedicated Nurse Practitioner with experience in home health to perform comprehensive risk assessments for Medicare Advantage plan members across Howard County, IN. This per diem position offers autonomy and a highly flexible schedule, ideal for those seeking work-life balance and the ability to tailor their weekly or bi-weekly hours. Shifts are available any day of the week, Monday through Sunday, with the freedom to create your own schedule.
Schedule:
Flexible shifts available Monday through Sunday Self-scheduling for your preferred days and hours Minimum weekly or bi-weekly hourly commitment
Primary Responsibilities:
Conduct annual home health risk assessments for Medicare Advantage members Assess prior and current health history, including review of medical conditions and medications Deliver patient education related to their conditions and prescribed therapies Identify potential gaps in care, offering actionable recommendations for follow-up No requirement for prescribing medication or developing treatment plans
Qualifications:
Active Nurse Practitioner license in Indiana Board Certification through AANP or ANCC Current CPR certification (BLS) Graduation with an MSN from an accredited nursing program At least 6 months of current NP experience preferred This role provides compensation of $80-$90 per completed evaluation along with mileage stipend and administrative time. You'll be performing assessments in the patient's home in locations throughout Howard County, including Kokomo, Greentown, and Russiaville. Each risk assessment is expected to take approximately 45 minutes.
