Deliver high-quality patient care as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members across Cass County, IN, including areas such as Logansport, Galveston, and Walton. In this per diem role, you'll enjoy the flexibility to create your own schedule with shifts available any day of the week. Each home visit averages 45 minutes and may include annual health assessments or post-discharge evaluations.

Key Responsibilities:

Conduct thorough home health risk assessments for Medicare Advantage plan members Review patients comprehensive medical histories, conditions, and medication use Educate members on their conditions and prescribed medications Recognize gaps in patient care and recommend appropriate actions for resolution No responsibility for creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Indiana Board Certification via AANP or ANCC Current CPR/BLS certification MSN from an accredited nursing school At least 6 months of recent Nurse Practitioner experience preferred
