Key Responsibilities:

Perform home health risk assessments, typically lasting 45 minutes per visit Evaluate prior and current medical history, medical conditions, and medication usage Deliver patient education on health conditions and prescribed medications Identify and communicate care gaps with suggested recommendations for resolution Focus exclusively on assessmentsno development of treatment plans or prescribing required

Qualifications:

Active Nurse Practitioner License in Indiana Current BLS/CPR certification Board Certification via AANP or

ANCC MSN

from an accredited nursing school At least 6 months recent experience as a Nurse Practitioner preferred
