Key Responsibilities:

Perform comprehensive home health risk assessments, each typically lasting about 45 minutes Review and evaluate prior and current medical history, ongoing health conditions, and medication use Provide educational support on health conditions and prescribed medications Identify potential gaps in care, offering recommendations for follow-up or resolution No requirement to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Indiana Current CPR certification (BLS required) Board certification through AANP or ANCC Minimum of 6 months current nurse practitioner experience preferred MSN degree from an accredited nursing school Enjoy a competitive rate of $80-$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
