A per diem opportunity is available for an experienced Nurse Practitioner with a background in home health assessments. This position involves traveling within Delaware County, INincluding areas such as Muncie, Yorktown, Chesterfield, and Center Townshipto conduct home health risk assessments for Medicare Advantage plan members. Your role will play a crucial part in supporting preventative health initiatives and improving patient outcomes within the community. Schedule & Hours Flexible shifts offered Monday through Sunday Ability to create your own schedule Minimum weekly or bi-weekly hourly commitment required Key Responsibilities Perform annual home health risk assessments during home visits (approximately 45 minutes per assessment) Review and document prior medical history, current medical conditions, and medication usage Educate patients on health conditions, medications, and general wellness Identify gaps in care and provide relevant recommendations to improve patient health There is no requirement for the creation of treatment plans or medication prescribing Candidate Qualifications Active Nurse Practitioner License in the state of Indiana Current CPR certification (BLS) Board Certification via AANP or ANCC Preferably six months or more of recent NP experience MSN degree from an accredited nursing school This role offers a compensation structure of $80-$90 per completed evaluation, in addition to a mileage stipend and administrative time consideration.
