Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members in their homes Review and document detailed medical history, chronic conditions, and current medications Educate patients on their health conditions and prescribed treatments Identify gaps in care and provide recommendations for closing those gaps Focus solely on assessmentsno developing treatment plans or prescribing medications required Travel locally within Tippecanoe County for home visits

Qualifications:

Active Nurse Practitioner license in Indiana Current CPR certification (BLS) Board Certification through AANP or

ANCC MSN
