Primary Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members Review and evaluate patients current and previous medical history, health conditions, and medication lists Provide educational support on health conditions and proper medication usage Identify any gaps in care, offering professional recommendations for resolution These assessments do not involve creating treatment plans or prescribing medications, allowing you to focus solely on evaluation and education

Qualifications:

Active Nurse Practitioner license in Indiana Board certified through AANP or ANCC Current CPR (BLS) certification

Preferred:
