Key Responsibilities:

Perform annual home health risk assessments in patients homes throughout Fayette County Review and assess medical history, including prior and current conditions and medication lists Deliver patient education on disease processes, conditions, and medication instructions Identify and address care gaps, offering recommendations to improve patient outcomes No requirements for prescribing medications or developing individualized treatment plans Engage with a variety of patients, including post-discharge and regular annual assessment visits

Qualifications:

Active Nurse Practitioner license in Indiana MSN degree from an accredited nursing program Board certification through AANP or ANCC Valid CPR (BLS) certification At least 6 months of current experience as a Nurse Practitioner is preferred Enjoy competitive compensation, earning $80$90 per evaluation, along with a mileage stipend and coverage for administrative tasks.
