Key Responsibilities:
Conduct home health risk assessmentstypically 45 minutes eachat member residences in Marion, Gas City, Upland, or other Grant County locations Review and document detailed medical, surgical, and medication histories Assess current health status and educate patients on chronic conditions and proper medication usage Identify gaps in care, offering relevant recommendations to enhance care continuity Visits may include annual assessments and post-discharge check-ins No responsibility for developing treatment plans or prescribing medications
Qualifications:
Active Nurse Practitioner license in Indiana Board Certification (AANP or ANCC) MSN from an accredited nursing school Current CPR certification (BLS) At least 6 months of recent experience as a Nurse Practitioner (preferred) This role features a competitive rate of $80-$90 per evaluation, along with a mileage stipend and compensation for administrative time.
