Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage plan members Review and document prior and current medical histories, conditions, and medication use Provide patient education on chronic conditions and medication regimens Identify gaps in care and offer practical recommendations for resolution No responsibility for developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Indiana Current CPR (BLS) certification Board certification through AANP or ANCC required Minimum of 6 months experience as a Nurse Practitioner (preferred) Graduate of an accredited Master of Science in Nursing (MSN) program Enjoy compensation of $80$90 per evaluation, with additional mileage and administrative stipends.
