Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members Review and document prior and current medical history, assess medical conditions, and evaluate medication usage Educate patients about their medical conditions and medications to enhance understanding and compliance Identify gaps in care and recommend actionable solutions to address them No responsibility for developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Indiana Current CPR certification (BLS) Board Certified through AANP or

ANCC MSN
