Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage members Review patients current and prior medical history, conditions, and medication usage Educate patients on their medical conditions and prescribed medications Identify and address gaps in care, offering clear recommendations for follow-up No requirement to create treatment plans or prescribe medicine

Preferred Qualifications:
