Key Responsibilities:

Complete annual health risk assessments for Medicare Advantage health plan members in their homes Review prior and current medical histories, ongoing conditions, and medication regimens Educate patients about their medical conditions and medication management Identify gaps in care and provide recommendations for improvement No requirement to generate treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Indiana Current BLS (CPR) certification Board Certification through
