As a Nurse Practitioner in this per diem role, youll autonomously conduct in-home visits for Medicare Advantage plan members residing in various communities such as Kokomo, Greentown, and Russiaville. Each evaluation averages approximately 45 minutes, with visits that can include annual assessments or post-discharge check-ins.

Key Responsibilities:

Perform thorough home health risk assessments on Medicare Advantage members Review and document prior/current medical history, chronic conditions, and medication regimen Deliver patient education related to medical conditions and prescribed medications Detect any gaps in care, recommending steps to address identified needs Focus exclusively on evaluationsno treatment planning or prescribing required

Qualifications:
