Job Description

Make a meaningful impact in patients lives by providing comprehensive home health risk assessments throughout Howard County. As a Nurse Practitioner in this per diem role, youll autonomously conduct in-home visits for Medicare Advantage plan members residing in various communities such as Kokomo, Greentown, and Russiaville. Each evaluation averages approximately 45 minutes, with visits that can include annual assessments or post-discharge check-ins.
Schedule:
Flexible scheduling between Monday and Sunday Ability to design your own schedule Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
Perform thorough home health risk assessments on Medicare Advantage members Review and document prior/current medical history, chronic conditions, and medication regimen Deliver patient education related to medical conditions and prescribed medications Detect any gaps in care, recommending steps to address identified needs Focus exclusively on evaluationsno treatment planning or prescribing required
Qualifications:
Active Nurse Practitioner license in Indiana Current CPR certification (BLS) Board Certification through AANP or ANCC Preferably at least 6 months of recent NP experience Master of Science in Nursing from an accredited institution As part of this assignment, you will receive a competitive compensation of $80-$90 per evaluation, in addition to a mileage stipend and administrative timerewarding your commitment, expertise, and efficiency.
