A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.

Descripción del puesto: As a Transitional Liaison Registered Nurse , you'll contribute to our success in the following ways: Identify patients who may qualify for hospice care. Follow the hospice referral, admission, and documentation process. Assess patients' physical, emotional, social, and spiritual needs. Work with care teams to coordinate patient transitions and services. Meet with patients and caregivers to discuss care goals and complete initial assessments. Explain hospice services so patients and families can make informed decisions. Educate staff and referral partners on hospice eligibility and the transition process. What is Required? Current State license as a Registered Nurse. Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing. 3+ years' experience in a clinical care setting Experience within Home Health or Hospice is preferred What's in it for you?
