Job Description

Home Infusion Nurse, RN Job Overview:
We are an established specialty home infusion pharmacy seeking motivated professionals to fill the role of Infusion Nurse as an
INDEPENDENT CONTRACTOR
(1099) with possibility to grow into a full-time position if desired. The ideal candidate (s) would be located and willing to travel in a geographic area of Wabash, IN. The Infusion Nurse is responsible for scheduling and providing home infusions for the medications that we dispense. The Infusion Nurse provides direct patient care for patients in the home or alternate site care setting. This is a vital role for the growth and success of our pharmacy operations.
Responsibilities:
  • Performs direct and indirect patient care in the home or alternate site setting.
  • Assess patient during initial visit for current medical and psychosocial needs.
  • Develop plan of care and treatment. Collaborates with pharmacy on multidisciplinary care plan.
  • Provides written and verbal updates to plan of care; communicates with team members about changes in patient's condition or orders.
  • Coordinating and scheduling home infusion visits around pharmacy deliveries and patient availability.
  • Serving as a communication bridge between pharmacy, patients, and providers.
  • Provide clinical education/support for pharmacy and patients.
  • Provides timely and appropriate documentation.
  • Keeps current regarding nursing regulations and community health issues within geographic service area.
  • Must maintain an active and unrestricted nursing license in current state of practice.
  • Ensuring maintenance of proper credentialing documents for nurses and agencies in your area.
Required Qualifications:
  • Associates degree or higher in nursing or healthcare administration
  • Minimum 1 years' experience in a clinical setting
  • EXCEPTIONAL
    Peripheral IV access skills
  • One year home infusion experience required
  • Understanding of standard clinical procedures, and nursing rules and regulations
  • Excellent communication/interpersonal skills
  • Strong understanding of medical terminology, and infusion supplies used in the home setting.
  • Strong organizational and time management skills
  • Valid driver's license and proof of insurance
Preferred Qualifications:
  • Experience and knowledge with the products and therapies provided.
  • CRNI, IgCN or other infusion credentialing.
  • Functional knowledge and use of CPR +.
