Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage health plan participants Review and document medical histories, existing medical conditions, and medications Educate members regarding their health conditions and prescribed medications Identify care gaps and recommend steps for resolution No requirement to develop treatment plans or prescribe medication

Qualifications:

Active Nurse Practitioner License in IN Current CPR certification (BLS) Board Certification through AANP or ANCC Minimum of 6 months recent NP experience preferred MSN degree from an accredited school of nursing This position offers compensation of $80$90 per evaluation, plus a mileage stipend and pay for administrative time, rewarding your expertise and dedication to quality home health.
