Core Responsibilities:

Conduct comprehensive home health risk assessments, approximately 45 minutes per visit Review and evaluate previous and current medical and medication histories Educate patients on their diagnoses and prescribed medications Assess for care gaps and offer practical clinical recommendations Perform annual assessments and post-discharge visits as needed No responsibility for creating treatment plans or prescribing medications

Candidate Qualifications:

Active Nurse Practitioner license in the state of Indiana Board Certification through AANP or ANCC Current BLS/CPR certification Master of Science in Nursing (MSN) from an accredited institution Six months or more of recent Nurse Practitioner experience preferred Enjoy earning $80$90 per evaluation, plus a mileage stipend and compensation for administrative time.
