Signature HealthCARE is seeking a compassionate Registered Nurse to deliver resident-centered care in our long-term and post-acute facility. The RN will assess, plan, implement, and evaluate nursing care, administer medications and treatments, coordinate with an interdisciplinary team, and educate residents and families. Responsibilities Perform comprehensive resident assessments and ongoing monitoring Develop, implement, and update individualized care plans Administer medications and treatments per provider orders and facility policy Collaborate with physicians, therapists, and interdisciplinary team members Document care accurately in the electronic health record Educate residents and families on conditions, treatments, and self-care Supervise and support CNAs and other care staff Respond to changes in condition and emergencies promptly Ensure compliance with safety, infection control, and regulatory standards Participate in quality improvement and resident-centered initiatives Required Skills Patient assessment Medication administration Care planning Wound care IV therapy Infection control Electronic health records (EHR) documentation Clinical triage and critical thinking Supervision of CNAs/support staff Interdisciplinary care coordination
