Deliver essential healthcare services in a rewarding, flexible per diem role focused on comprehensive home health risk assessments for Medicare Advantage plan members. In this position, you will conduct in-home visits throughout Fountain County, INincluding Attica, Covington, and Veedersburgensuring thorough evaluations and support for those most in need.

Schedule:

Flexible shifts available Monday through Sunday Choose your own working days and hours Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

Conduct annual and post-discharge home health risk assessments, each typically lasting about 45 minutes Evaluate medical history, current health status, and medication regimens for Medicare Advantage members Educate patients on medical conditions, treatments, and safe medication usage Identify any gaps in care and recommend resolutions to enhance patient well-being No responsibility for creating treatment plans or prescribing medications

Desired Qualifications:

Active Nurse Practitioner license in Indiana Board Certification (AANP or ANCC) MSN from an accredited nursing program Current CPR (BLS) certification At least 6 months of recent Nurse Practitioner experience preferred
