Job Description

Responsibilities:
Conduct initial and comprehensive resident assessments using the Minimum Data Set (MDS) and develop individualized care plans Lead and participate in care plan conferences to ensure comprehensive care Collaborate with the interdisciplinary team to develop and implement resident care plans Identify and prioritize resident needs, and coordinate care to meet those needs Maintain accurate and timely records of resident assessments, care plans, and progress Participate in quality improvement initiatives to enhance resident outcomes and care
Requirements:
Current licensure as a Registered Nurse in the State of Kansas or Compact Licensure Minimum 2 years of experience in gerontological nursing or a related field Strong clinical skills and knowledge of gerontological nursing practices Excellent communication, organizational, and interpersonal skills Ability to work autonomously and as part of a team Strong attention to detail and ability to prioritize tasks effectively
