Schedule:

Full-time, Monday through Friday Daily visit structure with probable on-call responsibilities Responsibilities include: Delivering direct nursing care and clinical interventions to home health patients Conducting thorough assessments and completing timely, accurate OASIS documentation Collaborating with physicians and interdisciplinary care teams to develop individualized care plans Providing wound care, medication management, disease monitoring, and patient/caregiver education Communicating any changes in patient condition promptly to the clinical team

Qualifications:
