A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Professional Home Health Care Agency, Inc. is committed to providing superior in-home care through a wide range of services including skilled nursing, rehabilitation, medical social services, personal support and homemaker services to our patients in their homes. We have open positions in Case Management for Registered Nurse Nurses in our Whitley branch office.
Job Responsibilities:
The Case Manager is responsible for reviewing medical records to assure quality control and best practices in standards of care. Case Managers assist in providing on-going educational programs on documentation, coverage requirements and other staff development programs and participates in the medical reimbursement process to ensure timely and accurate filing of patient claims.
Job Qualifications:
. Current RN or LPN license, with no sanctions, restrictions, or provisions, in the state of Kentucky. Four years of related experience in a clinical environment in Med/Surg, ICU/CCU or home health. Case Management, utilization review and supervisory experience preferred.
