Key Responsibilities:

Deliver comprehensive in-home nursing care and skilled interventions Conduct detailed patient assessments and complete OASIS documentation accurately and promptly Collaborate with physicians and other healthcare team members to create and implement individualized care plans Provide wound care, oversee medication administration, offer disease monitoring, and educate patients Update clinical teams regarding patient status and changes in condition

Qualifications:

Current and active RN license At least one year of recent home health nursing experience Demonstrated OASIS documentation expertise Proficiency in HomeCare HomeBase (strongly preferred) Reliable transportation and a valid drivers license Proven ability to manage a consistent, full-time visit schedule Why Consider This Opportunity?
