A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Job Description:
Job Summary The Case Manager manages a designated caseload to coordinate and complete timely assessment, planning, implementation and evaluation of discharge plans and care transitions across the continuum of care. Ensure optimum utilization of the patient's and the Health System's resources and perform these duties within the requirements of CMS and other external review agencies. A case manager manages, facilitates, and coordinates the necessary care activities and treatments, applying an approach to care delivery that is called the case management process. The case manager also manages the communication among the varied care providers and other essential parties internal and external to the health care organization. The case management process focuses on the identification of patients who would benefit from case management services, and the activities of assessment, problem identification, care planning, care delivery, monitoring, and evaluation of the care provided, specifically for its relevance to the needs of the patient/family, and for the health care team's ability to meet the desired outcomes and established goals.
