A rewarding opportunity is available for an experienced and compassionate Registered Nurse specializing in Home Health. This contract role focuses on delivering skilled nursing services to patients in their homes, where an RN can foster meaningful, patient-centered care. Schedule and Assignment Highlights:

Full-time, 13-week contract Monday Friday, 8:00 AM 5:00

PM Caseload:

2532 points weekly (35 patient visits daily) Field-based care in patients homes Daily travel within designated territory (mileage reimbursement provided) Electronic documentation with Home Care Home Base (HCHB) and use of OASIS for assessments

Key Responsibilities:

Conduct OASIS assessments and develop personalized care plans Deliver skilled nursing care, such as wound care, IV therapy, and managing chronic diseases Evaluate progress, promptly report changes, and collaborate with multi-disciplinary teams Educate patients and families on conditions, treatments, and health management Maintain timely, accurate clinical records within HCHB Participate in interdisciplinary conferences and ongoing education Ensure compliance with regulatory and agency standards

Qualifications:

Active RN license in the state of practice Minimum 1 year of recent home health RN experience (field preferred) Proficient in HCHB electronic documentation and OASIS assessments Strong clinical, assessment, and communication skills Reliable transportation, valid drivers license, and current auto insurance Ability to manage a field-based, independent work schedule BSN preferred; experience with 2532 point caseload and Medicare guidelines is a plus
