Job Description
Signature HealthCARE is seeking a compassionate Registered Nurse to deliver high-quality, resident-centered care in our long-term and post-acute facility. The RN will assess, plan, implement, and evaluate nursing care; administer medications and treatments; coordinate with an interdisciplinary team; and provide education to residents and families. Responsibilities Assess, plan, implement, and evaluate individualized nursing care for long-term and post-acute residents Administer medications, treatments, and IV therapies safely and accurately Monitor resident conditions, perform timely interventions, and communicate changes to providers and families Document assessments, interventions, and outcomes in the EHR in accordance with regulatory standards Collaborate with an interdisciplinary team to support resident-centered care plans and discharge planning Educate residents and families on conditions, medications, and self-care strategies Ensure compliance with infection control, safety, and quality standards Support and mentor CNAs and other team members, modeling compassion, dignity, and servant leadership Participate in quality improvement initiatives, audits, and care conferences Contribute to a faith-based, mission-driven culture focused on respect, inclusion, and continuous improvement Required Skills Patient assessment Care planning and care coordination Medication administration and IV therapy Electronic Health Record (EHR) documentation Wound care and pressure injury prevention Infection control practices Geriatric and post-acute clinical care Vital signs monitoring and clinical triage Patient and family education Regulatory and compliance documentation
